Health improvement happens when we work together.

Creating a healthier Grant County takes all of us. From healthcare providers to community members, educators to business leaders, changemakers to people with lived experiences, growers to transit providers—everyone has something valuable to contribute. The Coalition for Health Improvement (CHI) is where these voices come together to turn ideas into action.

We are dedicated to building a strong, collaborative network with diverse representation addressing the social conditions that affect the wellbeing of Grant County residents.

Our vision is to empower communities to promote positive change through unified partnerships and collaboration.

The Vital Conditions Framework recognizes that health is created by the conditions of daily life where we live, learn, work, and recreate. This approach moves beyond traditional healthcare to address the social, economic, and environmental factors that shape health outcomes. When these conditions are strong, people can thrive. When they're missing, health and wellbeing suffers.
